A sore throat is usually viral and improves in about a week with pain relief, fluids, and rest. Bacterial tonsillitis is less common. Antibiotics such as phenoxymethylpenicillin are considered when scoring tools and your story suggest they may help — not for every scratchy throat.
Difficulty breathing, drooling, a muffled “hot potato” voice, or being unable to swallow saliva can mean quinsy or a threatened airway and needs emergency care.

What is a sore throat?
A sore throat is pain or scratchiness when you swallow. Most cases are viral — part of a cold or flu — and improve within a week. The tonsils may look red or have spots. Bacterial tonsillitis is less common but can make you more unwell with fever and swollen neck glands.
Glandular fever (EBV) causes a prolonged sore throat in some young adults. Quinsy is an abscess beside the tonsil and needs urgent in-person care. A threatened airway (drooling, sitting forward, stridor) is an emergency.

Viruses cause most episodes. Streptococcus (“strep throat”) is the main bacterial cause considered for antibiotics. Irritants and reflux can also inflame the throat.

Eligibility
A typical sore throat in an otherwise well adult who can drink may be suitable. Children who cannot be assessed, and anyone with airway warning signs, are not.
May be suitable if
- A sore throat for a few days, you can swallow fluids, and you are not breathless
- You can use simple pain relief and rest
- You will seek urgent care if you cannot swallow saliva or breathing becomes hard
- You can tell us about penicillin allergy and whether you have a weakened immune system
May not be suitable if
- Stridor, drooling, sitting forward to breathe, or severe neck swelling — go to A&E
- You cannot swallow saliva, or one tonsil is hugely swollen with fever after several days — possible quinsy
- A sore throat lasting more than 3 weeks, or a persistent neck lump — GP two-week pathway
- You are immunocompromised or have had a splenectomy and feel unwell

Epiglottitis and quinsy can threaten the airway. Do not wait at home if you cannot swallow saliva or you are struggling to breathe.
A sore throat lasting weeks, especially in smokers, needs examination to exclude other causes.
- Go to A&E for breathing difficulty, drooling, or collapse.
- See a GP if symptoms last more than 3 weeks or you have a persistent hoarse voice.
- Do not give aspirin to children under 16.

Side effects
Antibiotics can cause diarrhoea and rash. Painkillers have dose limits.
- Nausea or diarrhoea Take antibiotics as directed. Seek help for severe or bloody diarrhoea.
- Rash Stop and get advice. A rash after amoxicillin can also occur with glandular fever.
- Stomach upset from painkillers Stay within licensed doses.
- Thrush Can follow antibiotics.

How to take or use the treatment
Always read the leaflet. Finish antibiotics if they are prescribed. The steps below are general.

Take pain relief regularly at first
Staying ahead of the pain helps you drink. Do not exceed licensed doses or double-up hidden paracetamol in cold remedies.

If an antibiotic is issued, complete the course
Space doses evenly. Seek help if you cannot swallow the tablets or you develop a severe rash.

Rest your voice and avoid smoke
Whispering can strain the voice more than speaking softly. Stay away from vulnerable people while you have a fever.

If you cannot swallow saliva, or breathing changes, do not wait until the pack is finished — seek emergency care.

When to seek medical advice
Most sore throats are short and viral. Airway complications are uncommon but time-critical.
Seek urgent help (A&E) if
- Stridor, drooling, sitting forward to breathe, or severe neck swelling — go to A&E.
- You cannot swallow your own saliva, or one side of the throat is extremely swollen with fever after several days — go to A&E (possible quinsy).
- A non-blanching rash, confusion, or you feel extremely unwell — go to A&E.
- Severe allergy after starting an antibiotic — go to A&E.
Contact a clinician or NHS 111 if
- A sore throat lasting more than 3 weeks, persistent hoarseness, or a neck lump that does not go — see a GP (two-week pathway if cancer is possible).
- You are not improving after 3 to 4 days, or you cannot keep fluids down.
- You may have glandular fever (prolonged fatigue, very large glands) — see a GP; avoid contact sport if advised because of spleen risk.
- You are immunocompromised, have had your spleen removed, or this keeps recurring — do not rely on repeated remote courses.
