Introduction

Treatment overview

Oral thrush (oral candidiasis) is a fungal infection of the mouth caused by an overgrowth of Candida. It can cause white patches, soreness and a bad taste.

It is more likely in people who use steroid inhalers, take antibiotics, wear dentures, have diabetes, or have a weakened immune system.

Private Pharmacy Care can review antifungal treatment such as nystatin after a consultation, where it is appropriate.

The condition

What is oral thrush?

Oral thrush (oral candidiasis) is an overgrowth of Candida yeast on the lining of the mouth. It often looks like creamy white patches on the tongue, inner cheeks or roof of the mouth that may wipe off and leave a raw, sore surface. You might notice a cottony feeling, altered taste, or pain with spicy or acidic food.

Candida often lives in the mouth harmlessly. It is more likely to overgrow after antibiotics, with inhaled corticosteroids for asthma or COPD, under dentures, in poorly controlled diabetes, in dry mouth, or when the immune system is weakened. It is not a sign of poor brushing on its own.

Private Pharmacy Care can review antifungal treatment such as nystatin oral suspension after a consultation, where appropriate. A clinician decides if medicine is needed. Completing a form does not guarantee a prescription. Always read the patient information leaflet. Recurrent thrush can be a clue to diabetes or another underlying issue that a GP should check.

Symptoms

Symptoms of oral thrush

Typical oral thrush is uncomfortable but you can still swallow. Difficulty breathing, drooling, or being unable to swallow saliva is an emergency. A mouth ulcer lasting more than 3 weeks needs examination for other causes.

Causes

Causes of oral thrush

Yeast overgrows when the usual mouth environment changes. Treating the trigger reduces the chance it comes straight back after antifungals.

Benefits

Benefits and uses

Antifungal liquids or gels act locally in the mouth. Treating thrush can ease soreness and help you eat and drink more comfortably while the infection clears.

  • Targeted antifungal treatment

Where suitable, medicines such as nystatin oral suspension can be considered after review.

  • Advice on inhalers and dentures

Rinsing after steroid inhalers and cleaning dentures reduces recurrence.

  • Private supply

If prescribed, treatment is dispensed and delivered discreetly.

  • When to seek extra care

We will tell you if swallowing pain, high fever, or a very sore mouth needs urgent in-person review.

Eligibility

Eligibility

Mild, typical oral thrush in adults can often be managed after a remote review. Severe, spreading, or recurrent infection may need examination and investigation of the cause.

May be suitable if

  • White patches or soreness consistent with oral thrush, especially after antibiotics or steroid inhalers
  • You can use an oral liquid or gel as directed, holding it in the mouth before swallowing if that is how the product is used
  • You will continue inhaler technique or denture hygiene as advised
  • You can tell us about diabetes, immunity, and other medicines

May not be suitable if

  • Difficulty breathing, drooling, or inability to swallow — go to A&E
  • You are severely unwell, immunocompromised, or infections keep coming back without a diagnosed cause
  • Suspected oral cancer signs (an ulcer lasting more than 3 weeks, unexplained lumps) — see a dentist or GP urgently
  • Allergy to the proposed antifungal

A clinician decides whether treatment is appropriate. Completing a consultation does not guarantee a prescription.

How it works

How it works

Nystatin and similar antifungals damage the fungal cell membrane. They need enough contact time in the mouth. Complete the course even if patches start to fade. Recurrence is common if the trigger (inhaler residue, poorly cleaned dentures, high blood sugar) is not addressed.

  1. Complete a consultation

Answer a short, confidential questionnaire about your symptoms, medical history and current medicines.

  2. Clinician review

A UK-registered clinician reviews your answers to check the treatment is safe and appropriate for you.

  3. Dispensing and delivery

If a prescription is issued, our pharmacy dispenses the medicine and sends it in discreet UK packaging.

Safety

Safety information

Oral thrush can be the first sign of poorly controlled diabetes or immune problems. We may advise a GP check, especially if this is not your first episode.

Keep the course as prescribed. Stopping early can allow symptoms to return.

  • Rinse your mouth after steroid inhalers and use a spacer if you have been shown how.
  • Take dentures out at night and clean them thoroughly.
  • Seek in-person care if you cannot eat or drink, or if symptoms worsen after a few days of treatment.
  • Tell us if you are pregnant or breastfeeding so the clinician can choose a suitable option.

Side effects

Side effects

Local antifungals are usually well tolerated. Stomach upset can occur if a liquid is swallowed.

  • Nausea or mild diarrhoea More likely if the dose is swallowed. Contact us if it is persistent.
  • Unpleasant taste Temporary. Complete the course if you can.
  • Mouth irritation Rare. Stop and seek advice if the mouth becomes much more sore.
  • Allergic reaction Swelling, rash or breathing difficulty needs urgent care.

Using treatment

How to take or use the treatment

Always follow the patient information leaflet for the antifungal you are given. Local treatments only work if they coat the mouth for long enough. The steps below are general.

How to take or use the treatment
  1. Shake liquids and measure the dose

Use the supplied syringe or spoon, not a kitchen teaspoon. Hold nystatin in the mouth for the time stated before swallowing, if that is how your product is used.

  2. Time it away from eating if advised

Some gels and liquids work better if you avoid eating and drinking for a short period after a dose. Take out dentures and clean them; medicine may need to be applied to the fitting surface as directed.

  3. Complete the full course

Stopping when the mouth looks better is a common reason for prompt recurrence. Keep using inhaler rinse and denture hygiene at the same time.

  4. Do not share spoons or bottles

While you have active sores, avoid sharing cups and cutlery if you can. Wash the measuring device as the leaflet says.

When to seek medical advice

Mild nausea can occur if a liquid is swallowed. Breathing difficulty, a mouth that is shutting, or suspected oral cancer signs need a different pathway.

When to seek medical advice

Seek urgent help (A&E) if

  • Difficulty breathing, noisy breathing, drooling, or inability to swallow saliva — go to A&E.
  • Severe swelling of the tongue, lips or throat, or a widespread allergic rash after an antifungal — go to A&E.
  • High fever, confusion, or you are too unwell to drink.
  • A rapidly spreading facial swelling or severe pain suggesting a deeper infection.

Contact a clinician or NHS 111 if

  • Patches are no better after a few days of correctly used treatment, or they keep coming back.
  • You have diabetes symptoms (thirst, passing lots of urine, tiredness) or you are on a steroid inhaler and this is a repeated problem — see a GP and review inhaler technique.
  • A mouth ulcer or red/white patch lasting more than 3 weeks, unexplained lumps, or loose teeth — urgent dentist or GP examination (2-week pathway if cancer is possible).
  • You are pregnant, breastfeeding, take warfarin, or have liver disease — check the antifungal choice before you continue.

Common questions

Frequently asked questions

Tap a question to see the answer. If you still need help, our team is ready to assist.

Is oral thrush contagious?

It is not treated like a typical contagious infection, but Candida can be passed in some close-contact situations. Do not share bottles or spoons while you have active sores if you can avoid it.

How long does treatment take?

Courses often last several days. Follow the product directions. If there is no improvement, you need a further review.

Can I still use my steroid inhaler?

Yes in most cases — but rinse, gargle and spit after each use, and review inhaler technique.

Does yoghurt cure thrush?

Diet is not a substitute for antifungal treatment when an infection is established. Keep eating a balanced diet and take prescribed medicine.

When should I see a dentist?

If patches do not clear, dentures fit poorly, or you have a long-lasting ulcer, arrange dental or GP examination.
