How it works

Triptans (for example sumatriptan) stimulate serotonin receptors involved in migraine pathways and can reverse some of the changes that happen during an attack. Take them as soon as you are sure it is a migraine, unless your leaflet says otherwise. Do not exceed the maximum number of doses in 24 hours.

Side effects

Triptans can cause short-lived sensations that feel alarming but are often not dangerous. Stop and seek advice if chest pain is severe or persistent.

  • Tingling, warmth or heaviness Common, especially in the chest, neck or limbs. Contact a doctor if chest pain is severe.
  • Dizziness or drowsiness Do not drive until you know how you react.
  • Nausea Can be the migraine itself or the medicine. Follow the leaflet.
  • Dry mouth or flushing Usually brief.

Always read the patient information leaflet for your triptan or painkiller. Do not exceed the maximum number of doses in 24 hours. The steps below are general.

  1. Treat once you are sure it is a migraine

    Take the dose as soon as the attack is recognisable, unless your leaflet says to wait. Using a triptan for every mild headache can lead to overuse.

  2. Follow the repeat-dose rules

    Some products allow a second dose if the headache returns after improvement, after a minimum interval. If the first dose does nothing, a second dose of the same triptan often will not help — contact us rather than stacking medicines.

  3. Do not mix triptans or use ergotamine too soon

    Leave the stated gap between different triptans. Tell us about antidepressants and other medicines because of interaction risks, including serotonin syndrome in rare cases.

  4. Keep a simple attack diary

    Note dates, possible triggers, what you took, and how often. If you need acute treatment on many days each month, you need a prevention review rather than more of the same.

Tingling, warmth or a heavy feeling after a triptan can be alarming but is often short-lived. Stroke-like symptoms and thunderclap headache are emergencies.

Seek urgent help (A&E) if

  • Sudden “worst ever” headache, head injury, collapse, seizure, or headache with fever and neck stiffness — go to A&E.
  • New weakness, facial droop, trouble speaking, or aura that is much longer or different from usual — treat as a possible stroke and go to A&E.
  • Severe or persistent chest pain after a triptan, or signs of a severe allergic reaction — emergency care.
  • Confusion, very high fever, or a rash that does not fade when a glass is rolled over it.

Contact a clinician or NHS 111 if

  • Attacks are becoming more frequent, or you are using acute medicines on most days — speak to a GP about prevention and medication-overuse headache.
  • Triptans help only a little, or side effects such as heaviness and dizziness are hard to live with — a different product or form may be considered, or you may need in-person care.
  • Migraine in pregnancy, or you have started combined hormonal contraception and get migraine with aura — seek clinical advice promptly.
  • Vomiting that stops you keeping any medicine or fluids down during repeated attacks.
