What is hay fever?

That causes sneezing, itch, a runny or blocked nose, and itchy, watery eyes. Tree, grass and weed seasons peak at different times in the UK, so your worst month may not match a friend’s.
Hay fever can worsen asthma and disturb sleep. It is not a cold: you do not usually have a high fever, and colds last a week or two rather than the same weeks every year. Persistent one-sided blockage, bloody discharge, or reduced smell with facial pain may be sinus or ENT disease instead.

Side effects
Modern antihistamines are usually non-drowsy but can still affect some people. Nasal sprays can cause dryness or nosebleeds.

Always read the patient information leaflet. Nasal steroids work best when used regularly, not only on the worst afternoon. The steps below are general.

Start the spray before peak pollen if possible
Daily use for several days builds benefit. If your nose is completely blocked, a short pharmacist-advised decongestant may help the steroid reach the lining — do not use decongestant sprays for more than a few days.

Aim the spray correctly
Gently blow your nose, keep your head upright, aim slightly out towards the ear on the same side, and avoid sniffing hard. This reduces nosebleeds.

Take antihistamines at a consistent time
Fexofenadine is often taken before food. Do not mix several sedating products. Stop and seek advice if you cannot stay awake or drive safely.

Treat eyes separately if needed
Use lubricating or anti-allergy drops as directed. Do not put nasal spray in the eye. Contact-lens wearers should check the drop leaflet.

Dry nose or mild drowsiness can occur. Breathing difficulty and anaphylaxis are emergencies. Hay fever should not make you gasp for air.
Seek urgent help (A&E) if
- Severe breathing difficulty, wheeze that is not responding to your asthma reliever, or blue lips — go to A&E.
- Swelling of the tongue or throat, or a widespread allergic reaction with dizziness — go to A&E (anaphylaxis).
- A painful red eye with vision loss or severe light sensitivity — emergency eye care.
- Confusion or collapse during a thunderstorm asthma episode — go to A&E.
Contact a clinician or NHS 111 if
- Asthma is creeping out of control in pollen season — see a GP; you may need an inhaled steroid review, not only hay fever tablets.
- One-sided blockage, bloody discharge, or facial swelling — ENT or GP assessment.
- Daily symptoms despite a regular nasal steroid plus an antihistamine used correctly — a clinician may consider a combination spray or specialist referral.
- Sedation or you cannot drive on your current antihistamine — switch to a non-sedating option with advice; do not add caffeine and hope.
